Recently moved?

If you have recently moved, it is very important that you tell us. This is because from time to time we may need to contact you. There is also a danger that we may be asked to return your medical records if other NHS bodies have tried to contact you by post.  This may leave you in the unenviable position of not being registered with a GP.

If you have moved, we need confirmation in writing, please remember to make it clear if other family members have moved with you or not. In the case of families splitting or children moving between parents, we may require confirmation from your ex-partner or spouse.

Please click below to print off the form and bring it in to us. Please post it or bring it in to us as soon as you have moved. These forms are also available at reception.

Practice Boundary

Like all practices, we can only see patients within a specific area. The main reason for this is that we cannot commit to taking care of people who live too far from the surgery, because if you are very ill and we have to come out and visit you, the time taken to do this can cause unacceptable waiting times or even cancelled appointments for our other patients.

Please see the practice to Boundary Map to see if you live within this area.
