New Patient Registration 
Subject to the eligibility criteria below, all patients who live within the practice boundary and are eligible for free NHS treatment are welcome to register at our surgery.
You can apply to join our practice by calling into the surgery between 8:30am - 5:30pm and ask for a registration pack. You can also print off and bring the form into us by downloading it here. Please ensure that you complete all of the questions on both sheets. This information is required so we have your medical history that may be relevant to your immediate care. It is kept strictly confidential.
When you bring these forms with you, please ensure that you also bring photographic identification such as a passport and proof of your address.
All new patients are offered a health check to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Am I Eligible for NHS Treatment?
Any UK citizen who spends more than 6 months of the year living in the UK, is eligible for free NHS treatment. Anyone who has moved to the area from the EU, or who has been granted residency for more than 6 months are, subject to providing appropriate identification also welcome to join our list. Visitors coming from abroad may be eligible for treatment. These rules are determined by the Department of Health and take into account where you live, the nature of the illness and how long you are staying in the UK.
Non NHS Treatment
If you are not eligible for NHS treatment we may still be able to see you on a private basis. In this case fees will apply. We will always advise you in advance how much this is are and why we can't treat you under the NHS.
Practice Boundary
Like all practices, we can only see patients within a specific area. The main reason for this is that we cannot commit to taking care of people who live too far from the surgery, because if you are very ill and we have to come out and visit you, the time taken to do this can cause unacceptable waiting times or even cancelled appointments for our other patients.
