NHS Summary Care Records

This Practice works with the HSCIC to provide better information for authorised healthcare staff treating patients in emergency or out-of-hours settings.  This Summary Care Record system shares information relating to your current medication, allergies and adverse reactions from your clinical records here.  If you wish to opt out of record sharing between this Practice and the SCR please contact the surgery.

Accessing your Medical record

Under the General Data Protection Regulation (GDPR) 2018 everybody has the right to see, or have a copy, of data we hold that can identify you.  You do not need to give a reason to see your data.

We will process all requests within 30 calendar days – or up to a maximum of 90 days for complex requests.

If you want to access your data you must make the request verbally or in writing.  If you wish a third party to have access to your data – such as a solicitor or insurance company – they must prove that they are acting on your behalf and we will verify that you wish them to have access to the data.  Under special circumstances, some information may be withheld.  If you wish to have a copy of the information we hold about you, please contact a member of our reception team.

All initial requests for your data will be provided free of charge.  We may charge a reasonable fee for further copies to cover administration charges.
